Federal Investigation into Titan Submersible Implosion Reveals Serious Safety Concerns at OceanGate

Investigation into Titan Submersible Incident Continues in Washington D.C.

Washington D.C. – A federal investigation into the implosion of the Titan submersible, which occurred during a dive to the Titanic wreck in June 2023, took a serious turn as more details emerged during a hearing this week. The hearing is part of an in-depth probe conducted by the U.S. Coast Guard, which aims to shed light on the safety practices employed by OceanGate, the company behind the Titan.

Former Director Critiques Company’s Safety Approach

On Tuesday, David Lochridge, the former director of operations at OceanGate, served as the sole witness in the second day of the probe. He voiced serious concerns regarding the company’s approach to safety, notably stating that his warnings were routinely disregarded in favor of the company’s CEO, Stockton Rush. Lochridge’s testimony indicated a culture within OceanGate that prioritized ambition and “ego-driven” exploration over the well-being of its crew and paying customers.

Lochridge expounded on his background with OceanGate, mentioning a troubling incident from 2016. During a previous expedition aboard the submersible Cyclops I, he recalled that Rush had recklessly piloted the craft towards the wreck of the Andrea Doria. This historic ship, which sank in 1956, presented numerous dangers due to deteriorating conditions. He emphasized that he strongly advised against the dive but was ultimately overruled by Rush, who reminded him, “Remember, I’m the CEO. You’re just an employee.”

Panicked Moments and Immediate Red Flags

As Lochridge recounted the journey, he waved red flags, indicating that the weather conditions were poor and the wreckage was crumbling. Despite these warnings, Rush separated the Cyclops from its support vessel in an unsafe manner, causing damage to the submersible before they even began their dive.

Once in the water, Lochridge insisted that Rush keep a safe distance from the Andrea Doria wreck. However, contrary to his requests, Rush repeatedly ignored his advice. Instead, he maneuvered the submersible in a way that led to a near catastrophic incident where it became stuck underneath the wreck. Lochridge described the scene, noting that Rush began to panic and questioned whether they had adequate life support onboard. As Rush hesitated to pass the controls to Lochridge, panic escalated among the passengers.

Controversy and Consequences Following the Incident

Eventually, with encouragement from a passenger, Rush was compelled to relinquish the controls to Lochridge, who successfully navigated them back to safety. However, the fallout from that incident strained their professional relationship. Lochridge testified that expressing his safety concerns only resulted in further pushback from Rush, who considered him an “anti-project” voice. The culmination of these tensions led to Lochridge’s firing in January 2018.

Whistleblower Actions and Industry Challenges

In the wake of his dismissal, Lochridge sought to blow the whistle on the safety issues at OceanGate by reaching out to the Occupational Safety and Health Administration (OSHA), which agreed to investigate. His testimony raises pertinent questions about the state of safety protocols in the rapidly growing industry of deep-sea tourism, an area largely unregulated by governing bodies.

The hearings are expected to last up to two weeks, with potential outcomes ranging from new regulations to possible criminal charges against the decision-makers in these high-stakes ventures.
